Commercial Fertilizer License Application

All commercial lawn fertilizer firms wishing to do business in the City of Oak Park Heights must be licensed to do so.

Please fill out the attached Commercial Lawn Fertilizer Application form and submit it to City Hall with a copy of all licenses currently held, a Material Safety Data Sheet, and the $100.00 fee.
